Author opt-in

One author's row in the Author Opt Ins table. Use inside the table on that page:

• author: name as the author goes by; page: wiki page or book to link if different; glowfic_id: the number in their glowfic.com/users/N address.
• public, logged_in (threads visible only to logged-in users), locked (access-listed or account-locked threads), nsfw: yes, notify, ask or no. Leave empty when the author hasn't said.
• coauthors: must agree, exempt (co-authors needn't ask), some exempt.
• notes: the author's own words. These are what counts; the other columns are a summary of them.
• confirmed: yes once the author has checked the columns.
